- Assess the need for and assist with the development of Advanced Directives and understanding the various treatment options and decisions selected.
- Assist in accessing community services available for health care needs.
Advanced Illness Planning improves the quality of life in the final years by reducing fears and anxieties. Instead of fear, patients and families are empowered by directing their rights to self-determination, informed consent and access to their chosen treatment options.
Advanced Illness Planning is offered as a free community service through the Circle of Life Hospice Foundation.
